Not something I've really thought of much because aside from the tac layout option it's not something that I've really needed to consider. I found this guy when browsing youtube videos - he's a quadraplegic COD player and has set up a petition to try and promote custom button remapping so that less abled people can make the most of games like this. He's also a comedian and a very funny guy (as is probably a necessity for any comedian). Check out some of his routine and the petition here:

The funny thing is that the coding for custom remapping is already in there. All that are needed are a few lines to enable it and MAYBE an interface for console controls if the current is incompatible (which I doubt).

I know for certain that it was possible on the PS3 version of COD4. Although, it was done on a modified console, and I don't know if it would work properly otherwise.

Maybe it is possible to do the same on MW2.
