What's the whole story? Will it get me banned by vac? If not, what's the best settings for it - when it was 800 too many games were unplayable, at 100 I had almost only good games but too many "host disconnected" etc in lobby. At 200 it's bearable both ways. Should I be changing isp already?

It will not get you banned by VAC, definitely.

It seems that you've already found the best value depending on your connection quality.

Changing ISP in the hope of a lower ping won't do much for MW2, the reason being is that it doesn't use a dedicated server setup, your game relies on the speed of someone's internet who lives 200 miles away from you (whereas dedicated servers have such quick internet connections your own internet is the bottleneck - thus improving your own ping would decrease your ping to the dedicated server virtually proportionally.)
